Roots 2026 Excellence Awards

Do you know a Community Health Worker who is always involved in much more than they are assigned to? Perhaps an outstanding CHW who goes above and beyond for the well-being of their communities? If so, the Arizona Community Health Workers Association (AzCHOW) would like to recognize and honor the service of three Community Health Workers in Arizona. Once again, we bring you the perfect opportunity to recognize them in our 2026 Roots conference through Awards of Excellence during our Luncheon on June 26th. Whether it's their dedication, determination, and selflessness, please bring the spotlight to that CHW/CHR/Promotor(a) who does it all (and much more) for their communities.

One nominee will be selected among all for each award - Three awardees in total.

The categories are:

Outstanding CHW Award 2026
Heart of the Community Award 2026
CHW Advocate Award 2026

Outstanding CHW Award 2026

The Outstanding CHW Award recognizes the outstanding performance and qualities of the CHW/CHR/Promotor. The awarded CHW is believed to excel in the expectations of what a Community Health Worker is and recognized as a leader and example to follow.

Heart of the Community Award 2026

This award acknowledges the qualities of the CHW/CHR/Promotor who has demonstrated compassion, empathy, and wholeheartedness towards their clients, community, and organization. The award honors them as the heart of the community.

CHW Advocate Award 2026

The CHW Advocate Award commends the efforts, qualities, and abilities of the CHW/CHR/Promotor who is self-driven, fights for the equity of their community, and demonstrates leadership skills that awards them the advocate of the year.
