About the Event

Training Description

This virtual training, hosted by AzCHOW in partnership with Banner Health Plans, is designed to

equip Community Health Workers (CHWs), Community Health Representatives (CHRs), and

allied healthcare professionals with the tools, knowledge, and culturally grounded strategies

needed to support diabetes prevention and management in Arizona’s diverse communities.


Scope of the Training:

Understand diabetes basics: including types, symptoms, and long-term complications.

Learn how movement acts as medicine: including traditional forms of physical activity and

ADA-aligned recommendations.


Explore core management tools:

Nutrition, medication adherence, and blood sugar

monitoring.


Recognize social and emotional barriers:

How to care, and how CHWs can help navigate

these challenges.


Strengthen your role as a trusted connector:

supporting patients in managing their health

and accessing local resources.


Apply real-world examples:

culturally responsive approaches to support better outcomes in

tribal and underserved populations.


Speakers:


Javier Gomez, MS, RD, CDCES


Javier Gomez is a Registered Dietitian and Certified Diabetes Care and Education Specialist with the Dial into Diabetes program. With a strong background in clinical nutrition and diabetes management, Javier brings a wealthof experience to his role. He is a retired military dietitian

and formerly served as an instructor at the U.S. Army Medical Center of Excellence, where he trained healthcare professionals in advanced nutrition and wellness practices. Javier is passionate about helping individuals and communities make sustainable, health-conscious choices. In his personal time, he enjoys staying active through a variety of activities including jogging, swimming, cross-training, and golf. Most of all, he values quality time with his wife and two children, often joining them in their many family adventures.


Amber Slade, Exercise Physiologist- Health and Wellness Coordinator


Amber Slade is an Exercise Physiologist with the Dial into Diabetes program, bringing over 17 years of experience in the fields of fitness, wellness, and psychology. Her diverse background includes roles as a psychology professor at a local university, published author, exercise science

instructor, and keynote speaker. Amber currently serves as the Lead Health and Wellness Coordinator for Ignite Your Health, a community-based support network for individuals managing diabetes. With a passion for holistic well-being, Amber integrates both physical health and mental

wellness into her approach. Outside of her professional work, she enjoys spending time outdoors—especially hiking and trail running—as well as being with her grandchildren and family. She also serves on her church’s praise and worship team, though she jokes not to ask her to sing solo.


Elizabeth Henriquez, Registered Nurse


Elizabeth Henriquez is a Registered Nurse with a decade of experience in patient care and healthcare leadership. She spent the first eight years of her nursing career in a long-term acute care hospital, working with critically ill patients. Throughout her career, Elizabeth has served in a variety of roles including Floor Nurse, Charge Nurse, Nursing Supervisor, Chief Nursing Officer, Case Manager, and Director of Wound Care. Elizabeth holds a Bachelor’s degree in Psychology from the University of Arizona and earned her Nursing degree from Pima Community College. She is deeply passionate about helping others and is committed to lifelong learning, especially in areas that support community health and wellness. Outside of her professional work, Elizabeth enjoys spending time with her family. She has been married for 10 years and is the proud mother of three beautiful children.
